Our office policies allow for us to provide the dental specialty treatment along with excellent customer service. If you have any questions or concerns, please feel free to contact our office.

Appointment Scheduling

Our office will always do our best to schedule appointments at your convenience.

Scheduled appointment times are made for each patient and we ask that you please contact our office 24 hours in advance of your scheduled visit if you are unable to keep your appointment. We realize that unexpected things may occur, but we ask that you assist us in this regard.

Finances

Dr. Malek can offer a number of flexible payment plans that will fit your specific timetable and budget. Most of our payment plans include monthly payment that continue throughout the treatment. We do not charge any interest and will also work to include your insurance benefits into your contract.

Insurance

We accept many Insurance plans. Begining with your first call to our office, we will ask that you provide us with all of the necessary insurance information and our Treatment Coordinator and Insurance Coordinator will work together to determine your insurance benefits and assist you in filing your insurance claim.

Our goal is to make sure that you receive the most from your insurance benefits. Please understand that only your insurance company can determine what benefits they will pay on a claim. We ask that you please keep our office informed about any insurance changes such as changes in insurance company name, policy name or change in employment so that we can process your insurance claims efficiently.
